Add MeshUtils submodule with radial refinement rings/center_cell and cell merging

Implements mesh refinement and coarsening utilities for UnstructuredMesh: structured radial refinement with concentric rings, cell merging, and moves all mesh manipulation code into a MeshUtils submodule.

MeshUtils submodule

• Moved refine_mesh from src/meshes/unstructured/refinement.jl into src/meshes/MeshUtils/
• Backward compatible — all functions remain directly accessible via using Jutul
• Also available as Jutul.MeshUtils.refine_mesh(...) etc.

refine_mesh_radial — radial refinement with rings and center cell

• n_rings: concentric ring subdivisions (default 1). Outer rings produce quads, innermost produces triangles to centroid.
• center_cell: when true, innermost region is a single polygon instead of triangular sectors.
• n_sectors: minimum angular subdivisions (existing, splits edges at midpoints if needed).
• Cell count per refined cell: S × R without center cell, S × (R-1) + 1 with.

merge_cells — cell coarsening

• Merges groups of cells by removing internal faces and cleaning up unused nodes.
• extra_out=true returns cell_map::Vector{Vector{Int}} mapping new cells → original cells.
• Works in 2D and 3D.

m3, info = merge_cells(m, [[1, 2, 5, 6], [3, 4, 7, 8]]; extra_out = true)
info.cell_map  # [[1,2,5,6], [3,4,7,8], [9], [10], ...]

Tests

• 480 radial refinement tests (volume conservation, normal orientation, cell counts for all parameter combinations)
• 102 merge_cells tests (2D/3D, multi-group, cell_map correctness)
• All 1520 existing refine_mesh tests unchanged and passing
