fix(release): scope physical UAT by device

Summary

• keep one complete canonical UAT protocol while applying its two documented Android TV Premium-profile cases only to TV
• require all 75 shared cases on phone and all 77 cases on TV without an operator-selected N/A path
• bump workbook, report, and physical-index schemas so old all-devices evidence fails closed
• bind the exact TV only documentation labels to the code policy in tests

Release impact

This fixes the evidence recorder before physical UAT. It does not alter the locked Bridge/Kodi candidate, deploy, APKs, or publication state.
