Docs: link canonical Tool Contract ecosystem

Summary

• Add a Tool Ecosystem table to the README so the kernel, contract, and planned organs are visible from the public entrypoint.
• Redirect stale in-repo Tool Contract specs to the canonical postmunnet/trinity-tool-contract repository.
• Update version lineage and spec indexes to distinguish Tool Contract ABI v1.0.0 from validation tooling v1.0.1.
