Make mostly-minimal changes to formatting of README.markdown to ensure that it is formatted correctly by GitHub. I've replaced the leading tab on each line of code with four spaces (but left other alone) and added a blank line before and after each code block. The only other changes I've made are to remove some trailing whitespace and duplicate blank lines.
One more small whitespace tweak to ensure section header are shown as intended.

The only other changes I've made are to remove some trailing whitespace and duplicate blank lines.
