feat(review): add curate action to promote a fact's provenance - #243

- canonic review gains a [c]urate action (alongside accept/reject/skip/freeze) that sets meta.provenance: human_curated on the resulting fact
- curate and freeze are independent and composable in apply_entry()
- closes the gap where an accepted-but-inferred fact stayed equally contestable by fresh inferred evidence forever, with no promotion path
- add specs/AMENDMENT-provenance-promotion.md (S17-S19); update SPEC-E4 §12 and the SPEC-E5-E15 meta comment accordingly
- update docs (ingestion-and-reconciliation, connectors, end-to-end-example, review-apply) to describe curate instead of hand-editing meta
- add tests proving promotion and the resulting CONTRADICTION on re-ingest
