What & Why
Release PR for v0.2.21 — the combined state-dedupe identity-fork repair (#159) and per-entry hook reconciliation (#161), plus their documentation records (#158, #160). Version files bumped, changelog converted from the curated [Unreleased] staging, artifacts rebuilt, and all three capability smoke receipts re-recorded against the prepared tree (claude-code 2.1.226, codex 0.147.0 on the spark pool, opencode 1.18.13).
