[codex] Add mixed-harness instruction drift record

What changed

• Adds a second deliberated internal-origin Koinara record: mixed-harness-instruction-drift.
• Reorders the About page agent-reading section so .md and llms-full.txt routes precede HTML discovery.
• Adds a short fresh-domain Markdown fallback cross-reference without duplicating the record.

Review

• Gemini consult approved Seed 10 as the strongest next record when framed as loaded-guidance probing rather than fixed vendor mechanics.
• Claude review approved the record body for public-safety, usefulness, stale-vendor-risk, tone, and structure.
