RUSTUNNEL_SERVER, RUSTUNNEL_API, and RUSTUNNEL_INSECURE now work as fallbacks for --server/--api/--insecure (flags still win). Needed by platforms that configure containerized MCP servers via environment only — Glama's Dockerfile builder, for one, rejects URLs in the start command. RUSTUNNEL_TOKEN already worked this way.
