Update MCP server to SDK v1.6.0#18

- Update `go.mod` to use `github.com/modelcontextprotocol/go-sdk v1.6.0`
and related indirect dependencies
- Adjust code to use new SDK types: `*mcp.GetPromptRequest`,
`*mcp.CallToolRequest`, `&mcp.StdioTransport{}`,
`&mcp.SSEClientTransport{Endpoint: ...}`
- Simplify return types across tools: `(*mcp.CallToolResult, any,
error)` instead of generic wrappers
- Fix test setup to use new `SSEClientTransport` constructor
- Fix test arguments: use `frameId` instead of `frameID`, and call
`disassemble` with `address` instead of `ip`
