These docs have changed to MkDocs, so the syntax needed to be updated as well as the paths/anchors. The extensions/concourse.py file has also been updated to reflect these changes.

This PR updates a number of broken links across the codebase. It also bumps the documentation dependencies.
The main issues were with the Concourse docs that have recently(ish) changed: see concourse/docs#572.
This required a change to the paths, anchors, syntax and a few other things to address these. As far as I can tell there is no easy (and well-supported) way to hook into MkDocs with Intersphinx.
I suggest we squash on merge.
