5th discovery strategy: when strategies 1-4 find 0 skills, check basePath/SKILL.md. Name from frontmatter, fallback to basename. relativePath set to "." for root skills.
8 new tests covering root SKILL.md detection, frontmatter name, basename fallback, relativePath ".", dedup guard when standard dirs have skills, references/scripts subdir copy, and installSource tracking.
