docs: record NUT-04 mint quote accounting design

Summary

This draft PR records the planning artifacts for adopting NUT-04 mint quote accounting in Coco.

It adds glossary language for Mint Quote Accounting, Remote Quote Update Time, and Mint Quote Reservation, and adds a proposed ADR for the cashu-ts/Coco quote normalization boundary.

Workflow

This PR is intended to support PRD issue #294. Implementation branches should either be based on this PR or incorporate these docs before landing the implementation. ADR-0006 should remain proposed until the implementation makes the decision true, then be accepted in the implementation PR.

Verification

• Docs-only planning change; no tests run.
