[codex] Refresh models and model option support

Summary

• Fix provider model refresh so the settings UI can reload provider model lists reliably.
• Replace hard-coded generation parameter rules with model metadata checks.
• Cache unsupported-parameter provider errors and retry without the rejected setting.
• Surface skipped model settings in the ClawPress settings UI, including cache-preserving behavior for selected models.
