allow component classes in content

• Allow compnent classes in content markup when prefixed with a dot, e.g. "This is Python: [.inline-code]import this[/]"
• Change to visual protocol, added RenderOptions to stop the signature from growing too much (breaking change)
• Moved Markdown COMPONENT_CLASSES to MarkdownBlock
