Complete Pi code-quality plugin port

• Complete the @sentiolabs/pi-code-quality package port for both slop-review and size-review.
• Add /code-quality-size prompt alias, size-review bundled exclusions, full package docs/metadata, and Release Please publishing wiring.
• Expand package tests to cover both skills, prompt aliases, bundled references, tarball contents, and Pi/source-fidelity guards.
