Honor ignored checks, explicit check selections, and required-only filtering before rejecting ambiguous GitHub App identities. Checks that actually participate in deployment policy continue to fail closed.
Add focused unit and bundled acceptance coverage for all three exclusion paths and mixed requiredness.
