Align status active/idle/closed -> viewed/engaged/clicked (ADR-001) and add optional focused_seconds/max_scroll_pct columns (ADR-0006) via an additive migration over the restored schema. Add schema/constraints pgTAP (status/additive CHECKs, UPSERT accumulation, cascades) and update generated database types.
