Document per-module test expectations (wallet, payments, transactions,
vault, config, errors) with copy-paste examples, explicit happy/negative
paths, and verifiable no-test justification rules. Addresses merged PRs
that shipped without sufficient test coverage.
Link the guide from README.md and CONTRIBUTING.md.
