Use CloudWatch alerts and notifications for health checks

Instead of active tests, use the AWS infrastructure to warn us of failed instances.

This should be implemented as a health monitor plugin. Since vpc-router is specialized on EC2, it's ok to do this as an internal plugin (within the vpc-router project).
