Add cart icon/link when items are in cart to header

Something like this. There could be a menu that pops up when hovered, but for simplicity sake, I think this simply linking to the checkout/cart page is pretty good. I imagine that when nothing is in the cart, this icon simply completely disappears. I believe this is how EDD does it.

Shouldn't be too difficult to accomplish outside of finding the necessary EDD functions. I made some slight customizations to .top-bar to get the nav centered around the Logo so there's a class .top-bar-right that holds the right-hand Nav items that it could potentially go into.
