Home Assistant version: 2026.5.3
Integration versions tested: v3.0.15 and v4.0.1-beta.2
Security provider: Frontpoint (powered by Alarm.com)
Summary:
Unable to complete setup of the Alarm.com integration due to 2FA authentication failures. The issue manifests differently between v3.0.15 and v4.0.1-beta.2 but results in failure in both cases.
Steps to Reproduce:
Install alarmdotcom integration via HACS
Go to Settings → Devices & Services → Add Integration → Alarm.com
Enter Frontpoint username and password
Integration accepts credentials and presents 2FA screen with SMS as the only option
OTP is never delivered via SMS
Behavior in v3.0.15:
Credentials accepted
2FA screen appeared with SMS as only option
OTP was received and entered successfully
Login resulted in "Unknown error occurred" and setup failed
Behavior in v4.0.1-beta.2:
Credentials accepted
2FA screen appears stating OTP has already been sent
OTP is never delivered via SMS
Frontpoint app security alert is eventually received on iphone through installed Frontpoint app stating "failed web login attempt" indicating Alarm.com is blocking the OTP delivery server-side
Integration never times out waiting for OTP entry, at least I couldn't wait more than 5 minutes.
Additional Context:
Direct login to alarm.com website works correctly and delivers OTP via SMS without issues
Direct login to Frontpoint website does not require OTP (trusted browser)
Home Assistant is already listed as a trusted device in Alarm.com account settings
The Frontpoint security alert confirms Alarm.com is intercepting and blocking the HA login attempt before delivering the OTP
Debug logs from 4.0.1-beta2 are attached. Logs after 15:03 on 5-23-2026 were after a clean restart
