Under INFORMED CONSENT..."You must inform survivor..." should "person giving information be added to this? The person’s information falls under the Individual Person information, if this isn’t clear let us know how to fix it. And so, the relationship to survivor/victim? If it is the “Reporting Party” then it will be recorded under that section. Again we need to add more text to the hint. What if the survivor is in no condition to provide information? Then I think the signature field will just have to be bypassed. For minors, their parents? Guardians? Good point, we may need to add a clause for this.
