Hide app name/icon on smaller screens

On smaller screens, the app name/icon should be hidden so that this doesn't happen:

Given that the length of the title is arbitrary in the first place, it should also ellipsize beforehand, but it should hide at some point. cc @pringelmann

I'd hide it on a small mobile.
Also there might be more buttons on the right side.
