lobby issues

• only host should be able to select game style
• when chosen team is changed, everyone should see it [v0.2]
• when a client which is ready is removed by the host, the “Ready” button should read “Ready” again
• same color / team warning should be more prominent
• client ready → hosts removes this client → client joins again → client still marked as ready @mabre
• client settings are ignored @mabre
• remove first team should not be possible @Jessypet

The “revenge goes back to lobby” thing we discussed is harder than I thought (Lobby needs another constructor which reuses existing client/server and gets the team information from wherever). I started to work on it (dd8cd00), but for v0.1 I simply disabled revenge (76cb1e0).
