Allow user to create a participant from intake form

Overview

A user should be able to fill out required fields on the intake form and click save. This creates a new participant.

Action Items

• Create a confirmation box that participant was created

• Add extra columns to the Participants table to account for new fields in intake form

• Add validations to required fields. If required fields are not filled in, scroll page to first unfilled required field.

• Have Save buttons disabled until all required fields are filled in.
