Checking the information in ignore_columns

It would be advantageous for the developer if the Active Record Doctor were to point out two shortcomings. Firstly, if a column within ignore_columns does not even exist in the DB, or secondly, if the column corresponds to the rules of the respective detector, even if it occurs in ignore_columns.

This applies not only to columns, but also models, tables. Instead of doing this, I think #114 should be done, so people can periodically regenerate the todo file with the actual information.
