Print the defendant's name on the court's part of the form(s) too?

Before adding this functionality, let's get validation from the D.A. or someone in the courthouse that:

1. this actually solves a pain point for the D.A./whomever, and
2. this wouldn't inadvertently cause problems for anyone's case (or for this whole endeavor).
