Ctrl+R startServer keybinding collision with browser refresh in codespaces

• VSCode Extension version (Check in VSCode Marketplace): v1.2.0
• Chrome
• MacOS

There are several issues like this with codespaces in general.

It's probably going to be tricky to work around:

• all possible exisiting browser shortcuts
• existing vscode shortcuts

and be left with any sensible options. Open to suggestions though!

One option would be to use the same shortcut for start/stop server - basically just toggles whichever one is not currently one. And then we'd only need one Ctrl+letter.

Not 100% sure that's possible though

I think between VSCode and the browser, pretty much every Ctrl+Letter is taken! (and sometimes more than once)
