Re condition-numbers, is there is a need to limit it to ten conditions? There may be circumstances where more condition changes than ten are sought.

Thanks @JBakerL - good spot, the current forms limit to 10, as in they have space for 10 but you are right it doesn't need to be limited to ten. I've updated the table.

Is the householder-development question new? I think it might confuse people - can't the LPA tell from the first application whether this is a householder application or not?
Is the NMA question to identify whether this is an amendment to a planning permission that has been amended by an NMA? If yes, an NMA amends a permission, unlike a s73. So original permission prior to the NMA cannot be implemented, only the amended version
