Web: Referrals + Chat window sizing

If the web window for a patient's page becomes too small, the chat space scales accordingly and becomes just as small.

Let's implement a minimum size for the chat, if the window becomes too small to support it, the user can scroll down so they get more space for the chat on their screen (as opposed to having the chat title and patient controls taking up half of the screen).

• Implement minimum vertical size for the chat + referrals page
• If the chat element is overflowing, let the screen naturally revert to scrolling
