add more detail to the Too High and Extreme HLT output bandwidth messages

As proposed by @Sam-Harper after a recent incident we should add the explicit HLT output rate and output bandwidth to the instruction part in classes HltOutputBandwidthExtreme and HltOutputBandwidthTooHigh.

Messages should be modified to something similar to:

    1. ...
    2. Check the per-stream bandwidths in F3Mon. Call the HLT DOC and tell him/her that the 
   HLT output rate is {{...}} and the HLT output bandwidth {{BANDWIDTH}}.
