Improve workflows between CRDCH model generation and CCDH Terminology Service

Currently, CRDCH model generation requires updating the CCDH Terminology Service, i.e.:

1. The model is generated from the Google Sheet.
2. The list of mappings to node data dictionaries needs to be sent to the CCDH Terminology Service.
3. The CCDH Terminology Server generates enumerations for all mapped fields with concept identifiers.
4. The model needs to be regenerated in order to incorporate the updated enumerations from the CCDH Terminology Service.

We need to work with the Terminology team to improve this workflow.

This has been planned, but we ran out of time to implement this this quarter, so I'm postponing it to the next quarter.
