Free text box when "Other" selected

Requirement

When selecting from dropdown list for predefined results the desired response may not be available but still needs to be recorded

Proposal

Have "Other" as last option for predefined results and if selected a free text box will appear for users to enter a response. Maybe we could add free text option to the Flag parameter and select it when we want free text box to appear when corresponding display value is selected.
