ledger method support for ledger_hash arguments

Currently ledger method only supports ledger_index arguments, rather than both ledger_hash and ledger_index.

Are you referring to a particular method, or all methods which are using ledger_index?

I was talking specifically for the ledger method, but looking through the Rippled API it seems that all ledger-related methods: ledger, ledger_closed, ledger_entry and ledger_data should take ledger_hash as an optional argument (when ledger_index is left empty).
