Add verifier assignment and multi-verifier (N-of-M) support

Currently there's no verifier assignment in the contract. On createGrant, accept a verifier address (or array of verifier addresses with a quorum threshold for N-of-M). Gate approveMilestone and rejectMilestone to only those addresses.
