Add writing ybar to VTK results files

Problem

SimVascular uses a VTK simulation results file with ybar data to perform adaptive mesh refinement; a five-component array containing average velocity, average pressure, and average speed written by svSolver.

svMultiPhysics does not currently write ybar data so the adaptive mesh refinement capability in SimVascular can't be used.

Solution

Add writing ybar data to simulation VTK results files.
