MCP tool suite profiles and per-agent exposure (S4)

Summary

Define named MCP tool profiles (e.g., "research", "execution", "exploration") and assign profiles to agents. Each agent only loads the MCP servers and tools it needs.

Why

Currently all MCP servers load for all agents. This wastes context budget and can cause tool name collisions or confusion.

Acceptance Criteria

• Named profiles defined in stack.toml or agent config
• Per-agent profile assignment
• MCP servers started only when needed by active agent
• Documentation for profile system
