Syncing a single file, e.g. via github action or cli, in my case in some sub folders, random filename, does lead to creating two pages in my notion database. Was using --save-id flag.
This does only happen when there is no id yet stored in the markdown file. An update of contents worked, if the id was already there from a previous sync.
