Proposed change. New docs/AGENT_LOOP.md describing the reason→act→observe loop, how tools
are advertised to the model, maxIterations/toolChoice semantics, the policy gate on every
tool call, and the emitted trace events. Update DESIGN_DOC.md §F/§G to reflect that agents now
call tools.
