Wait Time Submission Form

• Allows a user to submit how long they waited in court for their ticket
• Only allow 1 submission per ticket
• Pre populate the form based on the ticket number that was supplied
• Allow searching for a ticket if one wasn't supplied
• Provide a nice prompt if the ticket already had a time submitted
