Unknown URL showing in scan results

Describe the bug
I am seeing Unknown URL on a majority of the blockers identified. When filtered to HTML, I see 70 blockers and all have a URL. When filtered to PDF, I see 1036 blockers and all have a URL. When filtered to ALL, I see 12736 blockers and most of then show Unknown URL only 1106 have a URL.

To Reproduce*
Steps to reproduce the behavior:

1. Go to https://app.equalify.uic.edu/audits/3d56ba161dd849e6b46c041cad3b79b0
2. Scroll down to Audit Report
3. Switch to Detailed View
4. Unknown URL listings which if clicked go to a page which says page not scanned yet; clicking on the short code link of the blocker goes to blank page with 404 error at the very bottom.

Expected behavior
I would expect all blockers displayed should have a relevant URL

I also remembered that this was the same Audit what was related to my bug report of the multiple audits being created with the same creation date. Not sure if that may be related. If I should just try scanning again, let me know. For now I am leaving in current state, so you can look into it. Also I am currently scanning another site which has not been re-scanned yet.... will let you know if I have any similar Unknown URL results. So far, I think I have only seen one audit with this issue.
